Client Directed Home Care Invoice (CDHCI)
If you or your loved one needs eligible home care services and would like to choose your own approved home care provider, you can follow these steps:
1. Contact Alberta Health Services (AHS)
Call 811 and ask to speak with Home and Community Care.
You can say:
“I would like to request an assessment for Home and Community Care services. I am interested in the Client Directed Home Care Invoice (CDHCI) program because I would like to choose my own home care provider.”
2. Complete the Home Care Assessment
A Home and Community Care case manager will assess your care needs and determine whether you qualify for publicly funded home care services.
Let the case manager know that you are specifically interested in the Client Directed Home Care Invoice (CDHCI) program and would like to use a provider of your choice.
3. Once You Are Approved for CDHCI
After you have been assessed and approved for CDHCI, you can contact Gracewell Home Care Inc. to discuss your care needs and arrange your eligible home care services.
